Earlier this week, I had a really tough extraction that LOOKED easy. (Doncha just HATE it when they LOOK easy but end up like this one??)
ANYhow, during the luxation, the buccal plate came out with #2. This hasn't happened to me in over 10 years! I informed him before we did the ext that the tooth MAY be into the sinus.
ANYhow, I didn't really see that primary closure would occur with sutures so I packed him with gauze and dismissed him, and I didn't notice any sinus communication.
Later, the patient calls me and feels 'air leaking from the extraction.'
SO, Rx'ed him an ABO, (which made him sick of course!), and OTC decongestant. Called him later in week to see how he was. He felt like it was 'closed over.'
Yea, I know sectioning this tooth would have precluded this scenario, but darn it, I didn't think this would be a problem!
The kids teeth are awful, and he's addicted to Mt. Dew of course. I've restored many of them, but he's heading towards dentures. SO, no implant in that area.
Am I an awful dentist or what?? UG! Has this happened to any of you?
