RE: [ACEsthetics] Endo so skip if not interested

I have a photo of a stripped old Thermafill on metal carrier.

Yes and no.  Yes thermafil has some warm gutta percha properties.  No it is not the same.  The problems that I have seen with Thermafil is one of the GP being stripped off of the core on insertion into the canal and the operator being unaware of it.  It results in the apical portion of the root canal system not being obturated properly.  Part of the problem, and the reason the operator is unaware of it, is that the core has the same radiographic appliance as the GP around it so it still looks great on an x-ray.

I have retreated a number of these myself over the years and that is almost always how the core comes out of the canals.

BTW…I don't do retreats anymore.

Remember, I am slow, but if the gutta percha is warmed and flows and obturates the entire canal system would there be any difference in the result? Seems like we are essentially talking about GP flow, right?

System A is still GP (or similar) and sealer.  It is not a thermoplasticized piece of GP around a core.   System A is essentially a warm obturation technique.  It is freaking awesome too.  :]

How is this different from using Thermafil?

As some of you know I use an obturation system named after Jerry Avillion…System A.  It involves plasticizing the obturant be it a large single cone or cold lateral into a warm mass and condensing.  I did four teeth yesterday that are sort of ideal to his technique of taking a GT 20/0.08 one mm past the apex to fill that portion of the canal past the constriction with sealer but not totally destroying the constriction.  Note the condensed material and the sealer moving out the prepared canal after constriction.  I even got a bonus with a furcal accessory. We never found of cause of 6 going hot on us other than maybe traumatic occlusion. That GT made that curve on the distal of the molar out one mm.  I wonder where Jerry picked this up.  Jerry is back up and posting on IDF. Not practicing again but talking about it.   Guy
