We are kind of struggling with this right now also.  We have setup referal codes so we know what is going out but have no good way to track if the patient followed up.  We are using Eaglesoft.  As much as I love many things about Eaglesoft their specialist referal stuff is really completely useless.  It is like the programmer made it with no input from an actual working office.
