Re: [ACEsthetics] Apexification/apexigenesis

Right. IF pulp exposed, would have done some sealing after ozonation.

Usually MTA with GIC on top.

Actually not borne out by the experimental evidence once ozonated.

David, I never can find a good reason for leaving a hole in a tooth after years of experience watching bridge abutments be totally destroyed in six months when they come loose with minute exposure.  Imagine what leaving a massive hole allows.  The process of decay WILL start.

But I've gotta tell you that it would drive me NUTS having a big 'ol hole in my tooth like that with food getting down in there. You are correct about faster remin leaving it open. How close to the pulp do you come before closing it, Ray? Or don't you?

Because it will remineralize faster when exposed . & these generally aren't sensitive.

I know what you suggested works, Ray, but a hole like that in a molar of mine would drive me NUTS! Why not ozone, Caphosol, and then Fuji IX Extra as a temp and come back and do final restore later?

Also, what if the tooth is sensitive if you leave it?

I would do this.

Remove the gross decay, hit with ozone, DO NOT close up the cavity prep, give them some Caphasol, and have them apply it judiciously to the cavity prep 2-3 x/day for a month or so, bring the patient back in, fill with RMGI, and restore with composite of your choice.
You'll have a closed apex just as in the case I'm illustrating.

That's EXACTLY what this patient did!
Notice the closed apex and the additional amount of root formation that occurred after we 'apexified' the cavity prep? Ozone and caphasol. WONDERFUL stuff!

I have a small 7 year old little girl in the chair as a new patient.  She has severe decay on #3 & #19 that is into the pulp, but no obvious abscessing.  However, the roots are not totally developed with apical closure.  What would you all use to try and get these teeth to continue to develop to total apex formation?  Would you remove pulpal tissue from chamber totally?  What would you seal the teeth with restoratively?

Thanks .... Terry
