RE: [ACEsthetics] The Pinhole Surgical Technique

It appears to be a brilliant technique. Due to my course in nov, I could not go then. however, for anyone who might be interested, He opened up a course for Dec 6,7, allowing me to go then. J and there may still be openings. It is a brilliant technique and I think it will be just awesome to know. I spoke w him at length otp a few wks ago. v nice guy.

I signed up for the November course.  Will let you know more afterwards.  David Wong of Oklahoma uses  Perioderm beneath a full thickness flap for his coverage, and he gets results.  Danny Melker uses Perioderm with a split thickness flap, and he gets results. Difference in healing and blood supply?  None shown in their pictures. The Pinhole Technique uses a full thickness flap and detachment of the papillae around the tooth in question as well as at least one tooth on either side, so he flap can be coronally positioned.  Strips of Perioderm or Alloderm is then threaded through the access hole and guided to the area where the flap has been displaced coronally.  Sufficient amount of material is added (stacked) so that there is not a need for sutures (as the article says).  Healing s uneventful.  Dr. John Chao has developed specific instruments for this procedure.  I guess you could modify an Orban knife in order to get the flap reflected.  Getting the Perioderm into the correct position is done with a pair of modified cotton pliers.  Other than this, not familiar with the new procedure, besides it is useful for Class I and Class II recession defects.  Only slight success on Class III, due to bone level positioning.

This sure looks good IF, and I emphasize if, results stay stable.  There are many reports in the lit on ct grafts with advancement receding after a few years.  I know this is new, but have there been any long term followups.  And are there any training programs being offered.  These videos look great and minimal invasiveness.

It's interesting if you give your contact information and watch the videos of the technique, he's removing all decay and any restorations and basically leaving the notches in the teeth before bringing the gum tissue down. It would be interesting to see the long-term results of doing this. 

 

I like the technique and can see myself using it in my practice in many situations; and just like anything else...combining it with all the other aspects of dentistry that I currently know, occlusion, orthodontics, restorative, etc.

This would be worth learning more about. Truly innovative surgical technique and one that pts will certainly embrace. We ALL have pts who have recession issues and it would be great to see different results for different cases.
