In-spite of so called checks and balances, the quality of PhD theses is low because of the following:

1. To capture the promotion or senior post within the shorted possible  time with less efforts.
2. Sometimes both the supervisor and researcher are lenient.
3. Supervisor is after increasing the number of (score of) PhDs.
4. Sometimes the supervisor is incompetent.
5. Political influence.
6. Internet is a handy tool for cut-n-paste or plagiarism.
7. PhD theses are easily available in the market at an affordable  price.
8. Rules, regulations and procedures  are  often violated.
9. Editing is sometimes very poor.
10. There is a mind set: PhD can be earned, not necessarily to be obtained.
    
11. Opening of many research centres,a large number of institutes not having even a good number of teachers for UG/PG programs are running (PhD) research centres.
    
12. PhD is the faculty recruitment qualification for the post of Associate Professor, Professor and Principal.
13. Appointment of examiners, especially Indians within the country, give and take basis, I am your PhD examiner, you are mine, a nexus!
14. Underhand financial dealings, supervisors in universities almost get nothing for the efforts they put in. For my PhD candidate I being a  College faculty did not get  a single paisa for my efforts more than 4 years. A thankless job, whereas, university collected tuition fees for four semesters, 2 years minimum requirement.
    
15. Etc, etc.
