*Never judge tooth length by the size of your patient. This is a physician's wife and is at the very least tiny. She was phobic and came to me because I'd do everything in two appointments. This was originally a pulp cap that failed. We are going to do endo on 14 so I now know what to expect. The palatal measured 24 mm to buccal and MB and DB measured 23.5 to palatal cusp. Even the merging MB2 measured 22 to palatal cusp. This MB2 merged just short of POE for MB1 and the orifice was set very much to the palatal, palatal to the DB.
Canals were tiny and this is a combination of files. I need to get some 25 mm Blue Vortex 15/0.08 files. Instead I went to Protapers, using the SX to open orifices and the S1 and S2 to open canals. Then we went to Twisted Files and finished palatal to an 0.08 TF, MB and DB to 0.06 TF and MB2 to an 0.04 TF. I needed the flexibility to pick up the anatomy of the MB2 and the DB apex. This was picked up using a 10 C+ file. K3s in 0.06 and 0.04 were used to true up the variable taper.
Obturation was with EndoRez and was essentially a single cone technique on this tooth. The tooth was vital when we did the palliative. The cones totally filled the canals with no room for laterals to do System A so we left as is. Got a good fill. So here is a true single cone. Also, we bonded a temp crown on here because the tooth was so fragile. Ribbond bucco/lingual on floor. Hyperfill fill and bisacrylic temp. Guy
