Re: [ACEsthetics] How do you account for patients you refer out to specialists

No mirror.  The scope becomes the mirror substitute in your left hand.

I would think that you'd have to be close to right on top of the orifice to be able to see, because it would be hard to hold BOTH the camera and your mirror at the same time. I would think that positioning the camera right above the orifice would get in the way of a file. Often, it's tough getting those things in there all by themselves. No?

Anyone tried just using a half-decent intraoral camera held in the left hand as a substitute for using a microscope in endo?

Not great while water's spraying all over the place like when prepping a tooth, but endo??

Magnification can be similar to the heavy iron scopes and a heck of a lot easier to slew around to visualize things better.  Yeah, not 3D, but that turns out to be pretty irrelevant as soon as you put an instrument into the field.  The mind immediately snaps into 3D mode and understands exactly where stuff is.

Just kidding a friend.  It is very likely he bought it just for your office.  Lugging those things around would be a task I would not want unless you were built like a tank.  You could not use the base as a mobile part.  It would have to stay put…about 200 pounds.  But is does give him a spare to store off site if one of his goes down.  You probably have more room than him.  Are you in Dallas? I'd love to see some of the offices while I'm out there.

Well, it's certainly difficult to know intentions, and heck, maybe he left the one here that is on it's last leg for all I know. He has had it here for about 6 months, and we have to roll it into a closet when he isn't here. No one else uses it.

JIf he bought a scope just to leave at your office, he's lying a bit.  You don't buy a 20k dollar piece of equipment to sit for four days.  He must have one at his office on its last leg.  Now I'd have to buy one to leave at your office because I couldn't carry the scope in.  I'd buy a base but not the entire scope.  You can only get in about four or five endos in a day if they are molars and unless your fees are 2 grand a molar it isn't worth leaving one.  Jerking the chain of one of my favorite people.

Partner does wisdom teeth and some implants. Our resident periodontist does more implants and perio surgery. The endodontist works every Saturday (he bought a microscsope just to leave at our office), so there really is very little we refer out. Pediatric ortho - yep.

I know Marvin doesn't refer, but I also know you don't do anything involving tooth dust or blood, and your partner can't do everything, Kent! Implants, endodontics on troublesome patients, wisdom teeth- you know!

Referrals? What are referrals? I asked Marvin, and he had this puzzled look on his face, so I thought I would ask the group. Hey Dilatush, do you want us putting info on doctors not in our practice in our next mailer too? :-)

I am wanting to develop a form where I can know what happened with each patient I refer out to a specialist- surgeon, implant guy, endodontist, periodontist.

I fear the "black hole" syndrome, where you really don't know what happened with your folks, and sure, I can go look in practiceworks at a particular patient, but what if I don't remember that patient's name?

How do you handle this?
