[ACEsthetics] New Post/Thread Notification: General Dentistry

This 20 year old young woman came to my practice recently for a consult on trying to save her upper/lower anterior teeth. She has been seeing a periodontist for the last several years and according to her & her mother, this condition started around age 17. They were referred to the periodontist at that time and he has performed SRP and monitored her home care every month since then. At her last appt, he told her that she wouldn't have these teeth at age 21. There is no history of any "diagnosed" medical condition that could be contributing to this. She has been seen by a physician and has no hx of diabetes or thyroid issues. There are no hormonal changes of note. Pt claims no sinus problems or breathing issues, however she does have a small airway opening. No other medical conditions exist and she's not on any medication. Her diet consists of mostly healthy foods...fruits & vegetables and she doesn't drink any soda. Needless to say, she's embarrassed by her smile and wants it fixed, but she doesn't know what's causing it. Periodontal issues are confined to 7, 8, 9, 10 and 23, 24, 25, 26. See photos and x-rays attached.

What's your best guess at what's going on and what kinds of things would you look at in this case?

Would you consider doing ortho on this case to intrude the teeth to recapture some bone and then extrude them back into position?

Any thoughts/suggestions are appreciated. Thanks!
