Re: [ACEsthetics] Jeff Okeson on MAD's for OSA

Thanks Kent,
Yes it was a smaller study.  Not sure of the numbers and in my positive way I may have allowed my exuberance to show through with a strong statement like "NO" changes were noted.  I was just really encouraged that someone outside of the OSA field but with definite concern for the joint was not concerned by the treatment protocol long term.
Have a great day

Sorry, Dennie. I respect Okeson, but when you  described him as pretty highly regarded in these areas globally, it sounded like you were referring to OSA patients. I agree that he would be considered an expert in joints. The point I was making was that it had to be a small study, because you stated that In those cases which adhered to the protocol no changes were noted., and if you were to poll all of us who have been doing this for many years, we would all say that we have a number of patients who have been religious with their exercises, but who have indeed experienced occlusal changes.

Do you know how many patients were in the study?

Kent,
I don't remember referring to Dr. Okeson as an expert on OSA tx.  I do think he could be consider an expert on the Joint and much associated with it.  As we are changing the position of the condyle in the joint for extended periods of time it seems prudent to me that perhaps an expert on the Joint might be a good source to weigh in on the therapy that so many of us are offering our patients.
I did not state that there were NO changes.  His opinion was that much of the change seen was due to lack of compliance.

How long has Okeson been considered an expert on treating OSA? The study must be smallish, because my anecdotal reports from treating 20-30 every month says differently - I just had a patient in this morning with some changes, and he uses the CO bite splint every morning, and has been chewing gum like a madman every day, but after 3 months, can't get it back to normal. If anyone is "compliant" with exercise protocol, it would be this guy.

I specifically asked Dr. Okeson his thoughts on Mandibular advancement for the treatment of OSA.  He told me there is a study coming out soon and it revealed a positive message.  It was a smallish study but reflected 18% of the participants showed some occlusal changes.  NO joint issues.
He felt strongly that the occ. changes, which were in most cases open bite posterior, were attributable to poor compliance to am exercise protocol.  In those cases which adhered to the protocol no changes were noted.
Just wanted to share from someone that I think is pretty highly regarded in these areas globally.

My resistance to TAP's is based purely on personal experience- I had one and it really was uncomfortable, but it was a TAP2 and wasn't a great design, I think. I haven't tried any of the newer models.

maybe, maybe not.  TAP’s are very good.  I have found that it is simply a comfort thing.

I was just stunned at the improvement with  a TAP 3 Elite.  A Somnodent appliance might have been even better.
