To the best of my knowledge there is no copy right for questionnaire in isolation.Instead of entire questionnaire few of the items of each variable can be enclosed.

As per the rules and regulations of Bharathiar University( I also did my PhD there)  you can publish papers on the work in progress(say literature survey, development of scale etc) but not the entire thesis.
