As a recipient of treatment at a Shrine Hospital back from 1949 to 1961, I had not been exposed to their burns hospitals.  Later in life, when I became a member of the Shrine, I was on a club trip to visit the Shrine Burns hospital in Cincinnati.   This has been 25+ years ago and I'm certain that there have been many improvements; however, at the time we visited to "bring a little cheer", we were told by the staff MDs that most of these children's burns were either accidental in the home or inflicted by an enraged adult.   Wow!   I cannot imagine any adult doing that, but I guess we do lots of very bad things and this is just one more.  I guess we should pass a law checking the mental balance of anyone who will be boiling hot foods or water around our children.

Those burns on kids from chewing on electric cords can be extremely serious.  They seem to be OK, then the hemostasis fails on a significant number of 'em, and they can bleed out.

 

Not sure if it's just relaxation of the smooth muscle vessel walls or a bacterial suprainfection that has soft tissue necrosis effects on top of the direct heat damage.

 

These kids need intense supervision for the first 24 hours.  Also stents to prevent scar contraction.

Absolutely.  In my residency at Jefferson hospital, unless a patient specifically asked for an attending plastic surgeon, we WERE the plastic surgeons.  We were trained by the plastic surgeons to do plastic closures, and frankly it was a high percentage of what we did in the e.r.  lot of kids that would fall or get the crap kicked out of them (sometimes difficult to tell the difference).  And some of what we did was pioneering.  Anyone have a copy of kruger's oral surgery text?  Take a look at the chapter on oral electrical burns.  I had an unfortunate case when I was senior resident with a kid at least in theory biting into an electric cord and frying the commissure of the mouth.  At least that's what the pos mother claimed.  In any event, when we searched the lit, there was documentation on like 3 previous cases, so we took what info they had and then worked out the rest as we went along.  The photos in the chapter are all mine, in fact I hand waxed the appliance we put into place so the child could heal in a normal manner and have minimal disfigurement.  A number of years later, when kruger's text was being revised, they contacted me and asked permission to include our photos on a new chapter they were introducing regarding electrical burns.  My little contribution to the world.

I disagree. Oral surgeons are trained in "plastic" cosmetic closure. When I worked with residents it was a fight in the ER as to who got called for such cases... ENT, plastics or oral. The nurses would say that oral often had great skills and loved them.

 

I'd say if its a sun she could try to get one privately if she knows one otherwise she will need to go to the ER and request a plastic or oral max facial surgeon.
